Students must be logged in to their LAUSD mymail account to access this form.
Period or teacher change requests will not be considered. Students may submit only one request by August 16. No other requests will be considered after this date.
Students who are being evaluated for a sports team in the fall will have their schedules changed within the first few weeks of school once the coach has submitted their name for the team roster.
